What it means

Tennessee is close to the national TEFCA rate, and behind on HIOs

TEFCA went live in late 2023. Hospitals join through a Qualified Health Information Network, often one run by their EHR vendor or an existing national network, so the network a hospital already uses shapes its route in. In 2025, 81% of Tennessee respondents were on a national network, against 78% nationally, but only 62% took part in a state, regional or local HIO, against 83%.

8 of the 52 Tennessee hospitals answering in 2025 neither take part in TEFCA nor plan to; that group includes hospitals not aware of TEFCA and those that did not answer. For exchange vendors and HIOs, they are the open market.

Methods and sources

ONC/ASTP builds this file from the American Hospital Association Information Technology Supplement Survey of US non-federal acute care hospitals. Each hospital appears once, at the latest year it answered, so the file mixes survey years: 3,393 hospitals nationally and 68 in Tennessee (52 from 2025, 11 from 2022, 3 from 2023, 2 from 2024). TEFCA was not live for the 2022 survey, so older answers understate it. The headline compares Tennessee and the US using only 2025 answers; across all years, 34% of Tennessee hospitals and 34% of US hospitals report taking part.

Participation is self-reported. Tennessee has 52 hospitals in the 2025 survey, so one hospital moves the Tennessee share by about 2 points. Hospitals that did not answer a question count as not taking part. Planning to join counts only hospitals not already in TEFCA. A national network means CommonWell Health Alliance, eHealth Exchange or Carequality; the file does not split them. Hospital names and cities are matched by CMS Certification Number to CMS Hospital General Information; hospitals without a match show their CMS number. Hospitals that never answered the survey are not in the file.
