What it means
Federal certification requires a standard FHIR API, and nearly every certified EHR now publishes one. Endpoint counts reflect how a developer publishes, not market share: some list one endpoint per practice, others one per health system. The top three publishers account for 60% of all endpoints. Oracle Health publishes 1,320 (99.9% answering); Epic Systems Corporation publishes 776 (99.0% answering); Medical Information Technology, Inc. (MEDITECH) publishes 469 (69.7% answering).
Availability is where integrations break. Among developers with at least 100 endpoints, the lowest answer rates were Altera Digital Health Inc. (69.6%), Medical Information Technology, Inc. (MEDITECH) (69.7%), Veradigm, LLC (93.8%). Before you plan an integration, confirm that your EHR's endpoint answers, which FHIR version it runs and whether it publishes a SMART configuration for sign-in.

ONC's Lantern project collects the FHIR endpoint lists that certified health IT developers must publish and checks each endpoint every day. This page counts each endpoint once, using Lantern's default check, and includes only endpoints listed through the ONC Certified Health IT Product List. "Answering" means the endpoint returned HTTP 200 for its capability statement. "SMART config" means it also served a SMART on FHIR configuration for app sign-in.
